Municipal Contractor Licensing Forms

To submit an application for a Municipal Contractors License with the City of Arvada Engineering Division, you will need to download and return all of the completed forms from the related links.

You may also pick up the appropriate forms from our office or call us to have them mailed to you.

Please read the instructions page carefully. Completed applications and appropriate forms must be mailed in or hand delivered to City of Arvada, Engineering Division, 8101 Ralston Rd, Arvada, CO 80002. We do not accept faxed forms.

A Municipal Contractors License with the City of Arvada is good for one year. Renewal forms will be mailed out to you. It is up to you as a licensed contractor with the City of Arvada to make sure your insurance, worker's compensation and license bond are kept current.
